CONSENT PAGE
NABET 700-M UNIFOR (hereafter NABET) invites you to take part in a voluntary membership survey about your work experiences! This survey has been reviewed and endorsed by the Executive Board and the BIWOC (Black, Indigenous, Workers of Colour) Committee. Currently, NABET is collaborating with York University to carry out this statistical survey. Thus, the researchers from York University participated in the survey design and will contribute to the production of the research report.
This survey’s main objectives are to gather information to assist in developing our outreach, training and sponsorship activities, as well as track how the demographic of our membership is changing over time.
The information you provide is extremely valuable and will help the NABET leadership team improve union-based services and professional development programs to meet diverse needs of the members and enhance the working conditions for all. The survey’s main findings will be available for you to read once they are published on the NABET website.
You are being invited to participate in this survey because you have been randomly selected. This process of random selection helps us ensure better accuracy of the information we extrapolate during the reporting process. The subsequent sections provide information about what participating would mean, data collection and management, and respondents’ rights. Please read it carefully before deciding if you want to participate or not.
What is the structure of the Survey and what you will be asked to do in the Survey?
The survey link will remain active until Thursday, July 31, 2025. You will answer 47 multiple-choice questions. It will take you about 10-15 minutes to complete. This survey is conducted by Survey Monkey, an independent survey platform, which will not allow the researchers to link the survey answers to any respondents. These survey questions are divided into four sections.
-Part 1: General Background Information. These questions aim to collect members’ baseline information on topics such as educational background, income level, and length of employment in the screen industry. As well, this section will collect demographic data such as members’ language, gender, ethnicity, race, sexual orientation, and age.
-Part 2: Work Experiences. This section will ask you questions about your employment opportunities and professional experiences.
-Part 3: Participation in NABET 700-M UNIFOR. This section will collect information about how you connect with the union and your assessment of NABET’s existing services.
-Part 4: Workplace-Related Challenges. Questions in this section ask for more specific information about your experiences and concerns working on union productions.Voluntary Participation and Withdrawal
Your participation in this survey is completely voluntary. Your decision to participate or not is not connected to your member status or ability to participate in union activities now or in the future.
Participants may decline to participate by selecting "decline" at the start of the questionnaire. In this case, you will not be able to access the survey again and your data will not be included in the survey results.
Participants may start the questionnaire by selecting "agree" and finish at a later time. You can save your responses one of two ways - simply by closing your internet browser or by clicking the "EXIT" button on the top righthand corner of every survey page. When you return to the survey, be sure to use the same device to avoid duplicating data. You will resume the questionnaire after your last fully completed question page. Partially completed pages are not saved. Once a survey page is complete, you cannot go back and edit your responses.
You do not have to finish the survey in one sitting, however, if you do not complete the survey in its entirety before the submission deadline, you will not be able to access the survey again. Incomplete questionnaires will not be included in the survey results.
Once participants submit a completed survey, your data will be sent to SurveyMonkey and you will not be able to access the questionnaire again. Participants who submit a completed survey cannot withdraw and your data will be included in the results.Risks and Discomforts
You might feel uncomfortable by the questions addressing work-related challenges. You may decline to answer questions or may end the survey.
Benefits of the Survey and Benefits to You
Survey results will contribute to improving union-based services and professional development programs that can meet diverse needs of the members and enhance the working conditions for all. The survey’s main findings will be available for the members in the form of digital snapshots and a research report. Participants who complete the survey may enter a draw to win a $100 gift card. The URL displayed on the"Thank You" page will take you to a separate and unrelated survey where you will be asked for your contact details.
Confidentiality
You are asked not to include personal information about yourself or others in any of your responses. Only the York researchers and NABET Training and Program Coordinator, Bernice Manalo, will compile a list of members’ email addresses for distributing the survey via Mailchimp. During the survey, these researchers will not share any information on whether you choose to do the survey. After the survey draw is complete, the email list will be destroyed.
During the survey data collection, your email address is delinked by the Survey Monkey platform from the survey responses. Thus, the researchers cannot connect the survey answers to individual respondents. The information collected from the survey will only be used for research purposes. Survey results will be reported in a collective description in the research report and other publications. There is always a risk during web-based transmission that data can be intercepted by a third party (e.g., government agencies, hackers) and thus the confidentiality and privacy cannot be fully guaranteed. Please note that at the end of the study, NABET 700-M UNIFOR will keep the survey data in a collective description. No external researchers can access this data without NABET’s permission.

Your participation is instrumental in creating a better work culture now and for the future, so please make your best efforts to have your voice heard. This research has received ethics review and approval by the Human Participants Review Sub-Committee, York University’s Ethics Review Board and conforms to the standards of the Canadian Tri-Council Research Ethics guidelines.
